The Role

As a Nurse Practitioner on our team, you will be responsible for conducting thorough health assessments, reviewing medical histories, and developing personalized plans to address identified risks and needs. You will work closely with patients, families, and healthcare professionals to ensure seamless care coordination and optimal outcomes.

Responsibilities

- Conduct comprehensive health assessments, including medical history reviews, diagnosis, and treatment planning
- Develop and implement individualized care plans to mitigate identified risks and improve patient outcomes
- Collaborate with multidisciplinary teams, including physicians, case managers, social workers, and other healthcare professionals
- Provide education and support to patients and their families, empowering them to take an active role in managing their health
- Work independently and as part of a team to achieve shared goals and objectives

Requirements

- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
- Current licensure as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in the state of practice
- Board certification in a relevant specialty, such as Family Medicine or Gerontology
- At least 1 year of experience as a Nurse Practitioner, preferably in home-based or community settings
- Strong communication and interpersonal skills, with ability to build rapport with patients, families, and healthcare teams
- Proficiency in electronic health records (EHRs) and other relevant software applications
