About the Role

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. This is a part-time, PRN position that offers flexible scheduling and competitive compensation.

In this role, you will conduct in-home assessments for older adults and other at-risk individuals, providing comprehensive health evaluations and developing personalized care plans to improve their overall well-being.

You will work collaboratively with our interdisciplinary team to identify potential health risks and develop targeted interventions to address them. Your expertise and compassion will make a meaningful impact on the lives of our clients and their families.

Key Responsibilities:

  • Conduct home risk assessments, medical history evaluations, and comprehensive health screenings
  • Develop and implement individualized care plans to mitigate identified health risks
  • Collaborate with healthcare providers, family members, and caregivers to ensure seamless care coordination
  • Prioritize timely completion of assessments and documentation to meet business requirements

Requirements:

  • Masters degree in Nursing or commensurate experience
  • Current RN licensure in state of practice, including prescription authority
  • Board certification by AANP, ANCC, or AACCN in a Matrix-approved specialty
  • 1+ year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record software
  • Excellent verbal and written communication skills

Compensation and Benefits:
