POSITION: SOCIAL WORKER ASSISTANT

REPORTS TO: Social Work Supervisor, Hong Fook Centers

WORK SCHEDULE: 40 hours per week

EMPLOYMENT STATUS: Non-Exempt (3 months orientation period)

APPLICATION STATUS: Open until filled

POSITION SUMMARY: Under the guidance of the Social Work Supervisor, the Social Worker Assistant will deliver social work services to participants at the Community Based Adult Service Centers. Responsibilities include conducting intake assessments for potential participants, providing language support, collaborating with team members to deliver care management services, and assisting in the psychosocial evaluations of participants.

KEY RESPONSIBILITIES:

  • Introduce the programs of the Hong Fook Community Based Adult Service Center to potential participants and their families.
  • Conduct intake interviews, home visits, and initial assessments for prospective participants.
  • Facilitate the onboarding process for participants, including gathering necessary medical documentation, arranging transportation, and coordinating with team members.
  • Assist in the enrollment process for program participants, explaining program rules and participant rights, and aiding new participants in their adjustment.
  • Provide care management services to participants.
  • Support program participants in accessing necessary services and benefits, acting as a liaison with external agencies.
  • Interpret various service notices and medical insurance statements for participants and their families.
  • Assist the Social Work Supervisor in periodic evaluations of program participants and maintain accurate written records in compliance with licensing requirements.
  • Help facilitate support groups for program participants.
  • Engage in outreach activities targeting frail, elderly, and disabled adults.
  • Contribute as a member of the multidisciplinary team.
  • Perform other relevant tasks as assigned by the supervisor.

QUALIFICATIONS:

  • Bachelor's degree in Social Work or a related field.
  • Experience and interest in working with frail elderly or disabled populations.
  • Able to thrive in a diverse environment.
  • Capable of working independently and collaboratively within a multidisciplinary team.
  • Strong oral and written communication skills.
  • Public speaking skills are a plus.
  • Bilingual in English and Cantonese/Mandarin preferred; proficiency in other Asian languages (Vietnamese, Filipino, or Korean) is desirable.
  • Flexibility in work hours to meet program needs.
  • Possession of a valid CA driver’s license and access to a reliable vehicle is preferred.
  • Able to manage multiple tasks effectively.

ADA REQUIREMENTS:

  • Ability to communicate clearly with others.
  • Ability to see and hear to interact with others.
  • Ability to operate a computer.
  • Ability to lift a minimum of 20 lbs.
  • Vision 20/20 with correction.

SALARY RANGE: The target hourly rate for this position is $27.65, with benefits. The specific compensation will be determined based on the selected candidate's education, experience, and skill set.
