Job Summary

Pinnacle Home Care Inc. is seeking a skilled Physical Therapist to join our team of healthcare professionals. As a Physical Therapist - Home Health Care Specialist, you will be responsible for providing high-quality physical therapy services to patients in their own homes.

Key Responsibilities
  • Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  • Develop and implement personalized treatment plans in consultation with physicians and revise as necessary.
  • Collaborate with the interdisciplinary team to develop and implement client care plans.
  • Treat patients to relieve pain, develop or restore functional ability, and maintain maximum performance using physical means.
  • Teach, supervise, and counsel patients and their families in physical therapy programs.
  • Record and report evaluation data, treatments, and patients' responses to therapy interventions.
  • Prepare evaluations of patients' conditions and perform periodic reassessments.
  • Supervise Physical Therapist Assistants and Home Health Aides as required.
Requirements
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
  • Licensed or registered by the State.
  • At least one year of institutional experience.
  • Home care experience and supervisory experience preferred.
  • Acute/sub-acute setting experience required.
