Clinical Care Manager

**UW Medical Center-Montlake** is seeking a skilled **Registered Nurse Care Manager** to join our team. This role will be responsible for providing patient-centered care, coordinating transitions, and advocating for patients in our General Internal Medicine Clinic and OB/GYN and Primary Care Clinic at Roosevelt.
**Key Responsibilities:**
- Support patients in avoiding hospital admission/readmission by providing timely outreach, education, and goal-setting.
- Provide episodic and ongoing care management services for patients with complex needs.
- Collaborate with providers and staff across the UW Medicine system to ensure seamless transitions.
**Requirements:**
- At least 2 years of RN experience, preferably in primary care or internal medicine.
- Recent care management experience in an outpatient setting or nursing experience in a similar practice area.
- Active licensure to practice as a Registered Nurse in Washington State.
**What We Offer:
Salary Range: $76,960 - $134,798 per year.
