About the Job

We have an exciting opportunity for a dedicated Clinical Resource Manager to join our team at Swedish First Hill. As a RN PRN Case Manager Registered Nurse Home Health, you will play a vital role in ensuring that patients receive high-quality care and services.

Key Responsibilities
  • Coordinate patient care and services through the acute care episode, across the continuum.
  • Work collaboratively with interdisciplinary teams to improve patient care through effective utilization and monitoring of healthcare resources.
  • Identify and report cases and problems appropriate for secondary review to Case Management Department leadership, the Medical Director, or Physician Advisor.
