- Authorize Support and Services: Determine eligibility for support and services, ensuring that they align with individualized service plans.
- Manage Benefits and Services: Coordinate the management of benefits and services, ensuring that they are delivered in a timely and effective manner.
- Provide Advocacy and Resources: Connect youth and families with resources and services, empowering them to make informed decisions about their care.
- Complete Required Training: Participate in ongoing training and professional development to stay up-to-date on best practices and industry developments.
- Bachelor's Degree in a Health or Human Services-Related Field: Possess a degree in a field such as social work, psychology, or education.
- Experience Working with Youth and Families: Have at least one year of experience working with youth and families in a support or service capacity.
- Knowledge of Computer Applications: Be proficient in Microsoft Word and Excel, with the ability to learn other software applications as needed.
- Valid Driver's License and Insurance: Possess a valid driver's license and adequate insurance to meet agency requirements.
The Support and Service Coordinator will work in a busy office environment, with frequent outreach in the community and families' homes. The position requires flexibility, with some hours spent driving and working outside of regular business hours.
