Job Summary

We are seeking a highly motivated and compassionate Recovery Support Coordinator to join our team at the United Community Center. As a key member of our staff, you will provide comprehensive recovery support coordination services to clients, working closely with them to develop and implement personalized recovery plans.

Key Responsibilities
  • Maintain a caseload of clients and establish contact with them within 24 hours of referral.
  • Conduct needs assessments and develop Recovery Plans of Care (RPOCs) with clients, incorporating their self-identified goals and needs.
  • Collaborate with clients to form Recovery Support Teams (RSTs) and convene regular meetings to monitor progress and update plans as needed.
  • Provide individual face-to-face meetings with clients, connecting them with community resources and coordinating care among involved systems and providers.
  • Serve as an advocate for clients, collaborating with other systems and services to ensure their needs are met.
  • Complete and submit Service Authorization Requests (SARs) and Service Authorization Request Justifications (SARJs) as required.
  • Monitor client needs and refer them to other community resources or services as necessary.
Requirements
  • Associate's degree in Criminal Justice, Social Work, Human Services, or a related field.
  • Experience in case management or related community work preferred.
  • Knowledge of community resources and services, as well as the ability to work with diverse populations.
  • Valid driver's license and insured vehicle required.
Working Conditions

This position requires departmental business hours with the possibility of evenings and weekends as necessary. 24-hour emergency availability is also required. Travel in the community and home visits may be necessary.
