Role Summary:
The Patient Care Coordinator is responsible for ensuring the delivery of comprehensive care to patients by developing, monitoring, and assessing their home health care plans.
Key Responsibilities:
• Provide exceptional oversight of patient care delivered by home health aides.
• Schedule agency home health aides effectively, ensuring adherence to their assigned shifts.
• Manage unexpected absences by securing appropriate coverage promptly.
• Collaborate with Human Resources to establish a reliable on-call pool of aides across various shifts.
• Maintain open communication with referral sources and Case Managers to enhance customer service experiences.
• Assess client needs and align them with suitable caregivers.
• Oversee a caseload of over 100 patients, ensuring quality care and satisfaction.
• Review care plans collaboratively with patients and caregivers.
• Address and resolve any grievances or complaints from caregivers and clients.
• Monitor and manage the utilization of service hours effectively.
• Report any performance issues related to personnel.
• Ensure retention of caseloads and contribute to team objectives by achieving related results as necessary.
Minimum Qualifications:
• High school diploma or equivalent.
• 1-2+ years of experience in Home Care or Service Coordination (Required).
• Proven ability to maintain confidentiality and manage sensitive information.
• Strong skills in lead generation and referral monitoring.
• Proficient typing and computer skills.
• Ability to engage in business development and client acquisition.
• Highly organized, detail-oriented, and accurate.
• Excellent communication and public relations skills.
• Familiarity with HHA Exchange and EVV systems.
• Bilingual in English and Spanish is preferred.
