Service Coordinator-CHC Specialist

Location: Harrisburg, PA

We are seeking a Service Coordinator-CHC Specialist to join our Long Term Services and Support (LTSS) team. The ideal candidate will have experience assessing individuals needing personal care services and have knowledge of the home and community-based service system.

Key Responsibilities:

  • Conduct face-to-face visits to assess participant needs and monitor service delivery
  • Identify, coordinate, and assist participants in accessing needed LTSS services and other covered services
  • Lead the participant-centered planning and service delivery process
  • Inform participants about available services and support options

Requirements:

  • 3+ years of social service or related healthcare experience
  • Valid and current driver's license
  • Access to reliable automobile transportation with state-mandated minimum insurance
  • Ability to travel up to 90 minutes from home
  • Computer proficiency and working knowledge of MS Office
