Job Description:

As a Patient Care Coordinator, you will play a crucial role in enhancing the patient experience.

Role Overview:
As a member of the Care Coordination team, you will ensure that each physician practice session is thoroughly prepared and organized, facilitating patient visits with a focus on satisfaction.

Key responsibilities include:
  • Monitoring clinic schedules and making necessary adjustments to ensure smooth patient flow.
  • Acting as a liaison between patients, families, and the clinical team during office visits.
  • Expediting time-sensitive scheduling (within 48 hours) for urgent tests or consultations by coordinating with relevant departments.
  • Coordinating inpatient admissions when required.
  • Completing a detailed End of Clinic summary daily to reconcile work after clinic hours.

Key Qualifications:
We prefer candidates with:
  • Experience in customer or patient service.
  • A demonstrated commitment to healthcare through work, internships, or volunteer experiences.
  • The ability to manage acutely ill patients in high-pressure situations with a positive attitude.
  • Strong digital literacy skills.

Additional Information:
Work hours may vary between 6:15 am and 10:00 pm (Monday-Friday, with occasional weekends). Shifts are typically 12.5 hours, 3-4 days per week.

Schedules are provided one week in advance, and flexibility between shifts is expected.
