Key Responsibilities:
- Collaborate with a multidisciplinary team to develop and implement personalized care plans for patients.
- Assist patients in understanding their treatment pathways, ensuring they are well-informed and prepared for their appointments.
- Act as a liaison between patients and their healthcare providers, facilitating effective communication and support.
- Guide patients and their families through various healthcare settings, adhering to established care protocols.
- Employ strong communication skills to ensure that patients and caregivers are equipped with the necessary information to follow their treatment plans.
- Participate in initiatives aimed at improving quality and performance within the organization.
Qualifications:
- Bachelor's Degree in Nursing.
- Current Registered Professional Nursing License in New York State.
- Exceptional communication, interpersonal, and organizational abilities.
- Consistent professionalism and a keen eye for detail.
- Experience with Electronic Health Record systems.
- A minimum of two years of experience in an outpatient medical environment.
- Background in healthcare or customer service is preferred.
- Patient Navigation Certification is encouraged, with support provided for obtaining and maintaining this credential.
Work Schedule: Monday to Friday
Compensation: Up to $51 per hour, commensurate with experience. Our benefits package includes Tuition Reimbursement, comprehensive Health Insurance from day one, Dental and Vision coverage, Life Insurance, Short and Long-term disability, a 401k Plan, generous Paid Time Off, and eight paid holidays (including two floating holidays).
