Position Summary:
We are in search of a dedicated Bilingual Support Specialist focused on social services for seniors to operate across multiple community centers. Our offerings encompass case management, assistance with benefits, health education, and various supportive services. The Specialist will be instrumental in cultivating a nurturing environment for older adults through assessments, advocacy, and collaboration with community partners.
Key Responsibilities:
- Conduct interviews with clients to uncover challenges related to personal adjustments, financial issues, and health concerns, assessing the nature and extent of their needs.
- Deliver precise and relevant information regarding available services, benefits, and entitlements to clients.
- Facilitate on-site introductions for new participants to encourage engagement with existing program members.
- Maintain detailed case notes and develop actionable care plans based on client evaluations, following up to assess outcomes.
- Collaborate with program directors to ensure client needs are addressed with culturally appropriate solutions that effectively utilize community resources.
- Establish and nurture partnerships with other organizations to broaden resource availability; advocate on behalf of participants with external agencies.
- Engage in meetings, training sessions, and professional development activities.
- Attend relevant conferences and training opportunities to enhance skills and knowledge.
Qualifications:
- A Bachelor’s degree in Social Work, Sociology, Human Services, or a related discipline is preferred.
- A minimum of two years of experience in working with older adults or in case management within senior services is required.
- Familiarity with benefits and entitlements, along with experience in community organizing, is advantageous.
- Exceptional written and verbal communication skills are essential.
- Strong interpersonal abilities with a proven track record of building and maintaining relationships.
- Self-driven and resourceful, capable of working independently as well as part of a team.
- Ability to uphold confidentiality and exercise sound judgment.
- Proficiency in Microsoft Office Suite, including Word, Excel, Outlook, and database management tools.
- Bilingual proficiency in Spanish, both spoken and written, is mandatory.
