Job Summary

We are seeking a highly skilled Senior Behavioral Health Clinician to join our team at VNS Health. As a key member of our Behavioral Health team, you will provide supportive counseling, advocacy, education, and care management to help patients and their families navigate mental illness, access community resources, and manage symptoms to remain safely in the community.

Key Responsibilities
  • Provide direct care as part of a team, utilizing a client-centered approach to behavioral health care.
  • Develop and implement initial and ongoing clinical plans of care, ensuring effective care management for assigned caseloads.
  • Lead care coordination for complex psychiatric clinical cases, addressing client concerns and using motivational interviewing techniques to foster behavioral changes.
  • Develop inventories of resources that meet client needs, providing linkage, coordination, and referral to appropriate service providers and managed care plans.
  • Assist clients and families in developing sustainable networks of community-based supports, utilizing identified strengths and tools to prevent future participant crises.
Requirements
  • Master's Degree in Social Work, Psychology, Mental Health Counseling, Family Therapy, or related field.
  • Minimum of two years of mental health work experience providing direct services to clients with Serious Mental Illness (SMI), developmental disabilities, substance use disorders, and/or chronic medical conditions.
  • Bilingual skills may be required as determined by operational needs.
  • License and current registration to practice as a Mental Health Counselor, Marriage and Family Therapist, Social Worker, Clinical Social Worker, or related license in New York State.
