Job Overview

Community Health Action of Staten Island (CHASI) is seeking a highly motivated and compassionate Peer Advocate to join our team in Queens. As a Peer Advocate, you will play a vital role in supporting individuals living with HIV/AIDS, providing guidance, and promoting healthy lifestyles.

Key Responsibilities
  • Provide orientation and support to participants, helping them navigate our services and programs.
  • Accompany participants to provider sites, offering emotional support and navigation assistance.
  • Co-facilitate group activities, teaching clients how to identify high-risk thoughts and behaviors, and promoting prosocial behavior.
  • Assist with community outreach and program promotion, informing the public about available agency resources.
  • Provide food and nutrition counseling and education, helping participants make informed choices about their health.
  • Conduct follow-up sessions with participants, addressing barriers to care and treatment, and reinforcing retention in services.
  • Reach out to participants who miss appointments, ensuring they receive the support they need.
  • Attend staff development trainings, participate in supervision, and engage in team meetings to ensure seamless collaboration.
  • Complete program documentation in a timely manner, ensuring accurate records and effective service delivery.
Requirements
  • NYS AIDS Institute Certified Peer credential, preferred, or ability to obtain certification within the first 6 months of hire.
  • Lived experience with HIV, substance use, or mental health conditions, highly preferred.
  • High school diploma or high school equivalency, highly preferred.
  • Bilingual Spanish, highly desired.
  • Understanding of community-level work, collaboration, and coordination with other organizations.
  • Effective oral and written communication skills, ability to juggle priorities, and manage time productively.
  • Knowledge of community resources and organizations in Queens, organizational and problem-solving skills.
  • Proficient in Microsoft Word, PowerPoint, and Excel, familiarity with electronic health records or willingness to learn.
