About the Role

We are seeking an experienced and self-motivated Licensed Mental Health Therapist to join our team at Reema Health. As a key member of our integrated Care Team, you will play a crucial role in providing high-quality behavioral healthcare services to our members.

Job Summary

The Licensed Mental Health Therapist will be responsible for assessing and diagnosing behavioral health conditions, formulating treatment plans, and providing therapeutic interventions to our members. You will work closely with our multidisciplinary team, including Psychiatric Nurse Practitioners and Community Guides, to provide holistic care and support to our members.

Key Responsibilities
  • Assessment and Diagnosis: Conduct thorough assessments of behavioral health conditions, including chronic and acute conditions.
  • Treatment Planning: Develop and implement individualized treatment plans in consultation with other members of the Care Team.
  • Therapeutic Interventions: Provide evidence-based psychotherapy and other therapeutic interventions to our members.
  • Collaboration and Communication: Work closely with primary care providers, psychiatrists, social workers, and other professionals to discuss treatment plans and progress.
  • Documentation and Record-Keeping: Maintain accurate and timely documentation in our electronic medical record system.
Requirements
  • Licensure: LCSW, LMFT, or LPC with an active license in the state of Wisconsin.
  • Experience: 2+ years of outpatient experience working with adults enrolled in Medicaid, Medicare, or dual-eligible programs.
  • Skills: Excellent working knowledge of trauma-informed and evidence-based psychotherapy treatment for mental and chemical health conditions.
  • Technology: Proficiency with electronic medical records and other technology-based tools and systems.
