Position Summary:


The Housing Support Coordinator is tasked with aiding residents in establishing essential connections to ensure they can sustain a stable living environment that aligns with their preferences.

Key Responsibilities:
- Assist residents in finding safe and affordable housing options that meet their needs.
- Support residents in setting up and maintaining utility services.
- Aid in the acquisition of furniture and essential household items.


- Conduct regular monthly check-ins with residents to assess their living conditions and monitor their mental well-being.

- Document all interactions and visits in the resident's file.
- Employ person-centered approaches and maintain a compassionate demeanor when engaging with residents.
- Collaborate with residents and their service providers to deliver consistent support.
- Help residents resolve housing-related conflicts with landlords when necessary.
- Provide education, oversight, and recovery-oriented assistance in areas related to independent living.
- Work with residents to ensure all benefits and entitlements are secured and maintained.
- Collaborate with residents and their support teams to develop personalized housing support plans.
- Offer transportation assistance to residents as needed.


- Ensure residents access required clinical services and other necessary connections, including medical, dental, and vocational education.

- Facilitate the application process for HUD / Section 8 housing for all new residents.
- Maintain accurate records and reports as requested by management.
- Coordinate the move-in and move-out processes for residents as required.
- Perform any additional duties necessary for the effective operation of the program.

Qualifications:
- Bachelor’s degree or four years of experience in mental health or a related field, or an equivalent combination of education and experience.
- One year of experience in the human services sector is preferred; or an equivalent combination of education and experience.

*This position also requires a clean driving record as per company policy. The use of a personal vehicle is necessary. Candidates must be at least eighteen (18) years old.

Work Environment:
- This role operates Monday through Friday from 8:00 AM to 4:30 PM. There may be opportunities for hybrid work following a training period.
