Liberty Resources is in search of a dedicated Housing Support Coordinator to join our Safe Options Support initiative aimed at assisting the Rochester community.

The Housing Support Coordinator will play a vital role within a collaborative, multi-disciplinary team focused on engaging the unhoused individuals of Monroe County through a trauma-informed, person-centered, strengths-based, and non-judgmental approach. Our mission is to promote independence and well-being for homeless and unstably housed individuals who are often marginalized or underserved, providing compassionate care and connecting them to essential resources.

Position Overview:

This role will involve community engagement with a flexible remote/hybrid component. The schedule is Monday through Friday, with rotating on-call responsibilities. The primary objective of the Housing Support Coordinator is to assist individuals experiencing housing instability in securing permanent housing of their choice. Additionally, team members will support individuals in achieving their self-defined wellness goals. Ongoing education, supervision, and training will be integral to the Housing Support Coordinator's success at Liberty Resources.

Key Responsibilities:

  • Proactively reach out and establish relationships with displaced and unhoused individuals; engage participants in their environments.
  • Facilitate referrals to necessary care and support services through warm hand-offs.
  • Collaborate closely with existing care teams of individuals receiving services.
  • Monitor engagement in services and the status of permanent housing to ensure stability and well-being.
  • Conduct assessments related to mental health, addiction, and social determinants of health (training provided).
  • Implement crisis management and safety planning using established best practices.
  • Be flexible with rotating night and weekend shifts as needed.

Qualifications:

Ideal candidates for the Housing Support Coordinator position should possess a Bachelor's degree or higher in psychology, social work, sociology, or a related field, or be a New York State Licensed Practical Nurse (LPN). Relevant case management experience may substitute for formal education.

Preferred qualifications include:

  • Experience advocating for homeless or precariously housed populations.
  • Background in behavioral health, substance use, or homeless services.
  • Familiarity with community resources and support programs.
  • Understanding of peer support models and best practices.
  • Awareness of personal and systemic barriers to accessing care.
  • Bilingual in Spanish and English is a plus.

Required Skills and Abilities:

The successful candidate must be able to perform physical tasks such as sitting, standing, walking, bending, climbing stairs, and lifting objects up to 20 lbs. They should demonstrate a strength-based approach, professionalism, and the ability to build relationships and collaborate with other service providers.

Essential skills include:

  • Creative problem-solving and the ability to act with dignity and respect.
  • De-escalation techniques during crisis situations.
  • Sensitivity to cultural differences and the ability to incorporate this understanding into service delivery.
