Position Overview
Position Title: Service Coordinator
Employment Type: Full-Time
Location: Brooklyn, NY
Salary: $55,000 per year
Work Schedule
Days: Sunday to Thursday
Hours: 10 AM to 6 PM
Role Summary
The Service Coordinator plays a vital role in connecting clients with community resources, providing training in daily living skills, offering counseling and support, and ensuring compliance with program standards. This position serves as a crucial advocate for clients.
Key Responsibilities
Responsibilities include, but are not limited to:
- Facilitating connections for residents with available community services (e.g., case management, clinical services, vocational opportunities).
- Developing quarterly goal plans and conducting functional assessments.
- Meeting weekly with clients to enhance skills and track progress.
- Maintaining comprehensive documentation for client records (e.g., progress notes, discharge plans).
- Providing individual counseling and support services.
- Assisting with housing-related maintenance issues.
- Delivering training in daily living skills (e.g., medication management, financial literacy).
- Conducting monthly fire safety inspections.
- Participating in weekly supervision and staff meetings.
- Staying updated with all agency compliance requirements and training.
- Performing additional duties as assigned by the Program Director or Supervisor.
Compensation and Benefits
Salary: $55,000 per year
Qualifications
A Bachelor's Degree in Psychology, Rehabilitation, Social Work, or a related mental health field is preferred, along with a minimum of two years of experience in a similar environment or working with individuals with mental health challenges.
The ideal candidate will possess strong verbal and written communication skills and demonstrate the ability to make informed decisions regarding client care.
