Position: Service Coordinator
Type: Full-Time
Location: Brooklyn, NY
Salary: $55,000/year

Work Schedule:
Sunday to Thursday; 10 AM to 6 PM

Role Overview:
The Service Coordinator plays a crucial role in connecting clients with community resources, providing training in daily living skills, offering counseling and support, ensuring proper documentation, and adhering to program standards.

Key Responsibilities:

  • Facilitate client access to community services such as case management, clinical support, vocational opportunities, and entitlements.
  • Develop and review quarterly goal plans and functional assessments.
  • Conduct weekly meetings with clients to enhance skills and monitor progress.
  • Maintain comprehensive documentation for client records, including progress notes and discharge plans.
  • Provide individualized counseling and support services.
  • Assist with housing-related maintenance issues.
  • Deliver training in daily living skills, including medication management and financial literacy.
  • Conduct monthly fire safety inspections.
  • Participate in weekly supervision and staff meetings.
  • Stay updated with all agency compliance requirements and training.
  • Perform additional duties as assigned by the Program Director or Supervisor.

Compensation:
$55,000/year

Qualifications:
A Bachelor’s Degree in Psychology, Rehabilitation, Social Work, or a related mental health field is preferred, along with two years of experience in a similar environment or working with individuals with mental health challenges.

The ideal candidate will possess strong verbal and written communication skills and the ability to make informed decisions regarding client care.
