As a Resident Transition Specialist, you will play a crucial role in ensuring that the move-in experience for residents and their families is seamless and characterized by exceptional customer service. You will also be responsible for ensuring compliance with relevant housing programs.

Primary Responsibilities:
  • Handle inquiries from hospitals, families, and other sources, providing informative appointments that include warm welcomes, presentations, tours, and effective closing techniques.
  • Maintain model rooms for tours, ensuring they are ready and appealing based on bed availability.
  • Respond to inquiries promptly and ensure thorough follow-up.
  • Engage in follow-up communications, including phone calls, personal letters, and on-site meetings with family members.
  • Collaborate closely with the Director of Marketing.
  • Work alongside the Leadership Team to facilitate efficient decisions regarding prospective residents.
  • Stay informed about unit availability and coordinate with Nursing and Environmental Services for optimal unit selection.
  • Participate in daily meetings to keep staff updated on anticipated move-ins.
  • Contribute to the quarterly marketing strategy.
  • Exhibit outstanding customer service skills consistently.
  • Stay updated on changes to housing compliance programs.
  • Ensure that resident and applicant files meet compliance standards.
  • Engage in ongoing training related to housing program compliance.
  • Process and verify all income and asset information for resident move-ins and annual recertifications.
  • Prepare and submit annual reports as required.
  • Perform additional duties as assigned.
Qualifications and Skills:
  • Background in healthcare or related fields is preferred.
  • Understanding and sensitivity to the needs of seniors.
  • Professional demeanor with strong communication skills for engaging with potential residents and their families.
  • Knowledge of reimbursement processes and managed care referrals.
  • Ability to understand and assist with marketing materials.
  • Strong listening skills and empathy towards others.
  • Effective verbal and written communication abilities.
  • Capacity to build professional relationships with diverse individuals and teams.
  • Self-motivated and goal-oriented with the ability to work independently.
  • Analytical skills to identify problems and derive valid conclusions.
  • Flexibility in scheduling, including availability for occasional evenings and weekends.
  • Proficient in Microsoft Office Suite and compliance-related software.
  • Current compliance certifications related to housing programs.
  • Detail-oriented approach to compliance and reporting.
