Job Summary

We are seeking a highly motivated and experienced Case Manager to join our team at Women In Need, Inc. The successful candidate will be responsible for providing case management services to clients in transitional housing, ensuring their successful integration into both housing and employment goals, and ensuring compliance with Win's contractual requirements with outside funders.

Key Responsibilities
  • Conduct initial assessments of clients within 48 hours of arrival, completing the Family Case Assessment Form, Initial Service Plan, and all initial intake documentation.
  • Initiate referrals for necessary emergency services, ensuring timely and accurate input of all required client information in the Uniform Client Chart.
  • Collaborate with Housing Coordinators to ensure clients are compliant in seeking permanent housing, attending shelter meetings, agency-wide meetings, and staff training as directed by the supervisor.
  • Prepare all mandated reporting as required by supervisor, contract agency, and funding agency, conducting client unit inspections and reviewing findings with the client and supervisor.
Requirements
  • Bachelor's Degree in social work or related field preferred, with two years of relevant experience or an Associate's Degree and three years of client-facing experience.
  • Familiarity with Evidence-Based Practices (EBP) such as strength-based case management, motivational interviewing, trauma-informed care, critical time intervention, family-centered case management, boundary setting, and cultural-reflexivity.
  • Experience working with families and issues of domestic violence and homelessness, with excellent organizational, written, and verbal communication skills.
  • Ability to work effectively in a team environment, with computer skills and knowledge of CARES a plus, and bilingual English/Spanish proficiency a plus.
