Job Summary

We are seeking a highly skilled Care Coordinator to join our interdisciplinary team at the Institute for Comm Living. The successful candidate will provide care coordination to a caseload of severely mentally ill adults with multiple medical comorbidities and/or co-occurring substance abuse disorders and/or medically ill individuals.

Key Responsibilities
  • Conduct comprehensive assessments of assigned clients to identify strengths, needs, goals, and resources.
  • Develop and implement client-centered care plans in collaboration with other team members to ensure focus on desired outcomes.
  • Maintain effective communication with clients, primary care physicians, substance abuse, and mental healthcare providers, family, collateral resources, and other Agency staff on behalf of clients.
  • Coordinate care planning with other providers of services/resources to ensure goal-directed, collaborative care, including care transitions.
  • Participate in team meetings to provide input/feedback around psychosocial and medical conditions/comorbidities to review client status, update plans and goals, and review outcomes to further program goals.
Requirements
  • Bachelor's degree in a qualifying field or equivalent experience.
  • Two years of experience in providing direct services to individuals with Serious Mental Illness, developmental disabilities, or substance use disorders.
  • Excellent interpersonal skills and ability to work effectively in a team environment.
  • Working knowledge of computer software and electronic health record systems.
Preferred Qualifications
  • Master's degree in a qualifying field.
  • Knowledge of Medicaid, Social Security, and other entitlements.
