Clinical Liaison RN

This role represents Providence Home Services (HS) at contracted facilities, requiring strong communication and interpersonal skills. The Clinical Liaison RN coordinates and arranges home care services between HS and contracted facilities for home health, home infusion, and home medical equipment associated with a clinical referral.
Key Responsibilities- Coordinate home care services between HS and contracted facilities
- Arrange home health, home infusion, and home medical equipment services
- Communicate effectively with healthcare professionals and patients
- Ensure seamless care transitions and patient satisfaction
- Registered Nurse License in state of hire and wherever care is delivered
- ~1 year Full-time experience in clinical role
- Bachelor's Degree in Nursing
- Healthcare industry experience
- Documented prior successful sales account medical management experience
- Epic EMR experience
