Key Responsibilities- Coordinate and arrange home care services between Providence Home Services and contracted facilities
- Develop and maintain relationships with contracted facilities and healthcare providers
- Ensure seamless transitions of care for patients
- Collaborate with interdisciplinary teams to provide comprehensive care
- Monitor and evaluate the quality of care provided
- Registered Nurse License in state of hire and wherever care is delivered
- ~1 year Full-time experience in clinical role
- Bachelor's Degree in Nursing
- Healthcare industry experience
- Documented prior successful sales account medical management experience
- Epic EMR experience
