Position Overview

Role Summary

As a Nurse Care Coordinator, you will play a vital role in providing comprehensive care management. This includes conducting in-home evaluations, creating care plans, facilitating services, advocating for members, and authorizing necessary health services to address individual health requirements while ensuring effective and quality outcomes.

Key Responsibilities

  • Collaborate with members and their families to promote independent living.
  • Ensure continuity of care across the healthcare spectrum by assessing and monitoring members' needs and conditions.
  • Authorize covered services and coordinate care across various payers.
  • Engage with members, families, caregivers, primary care providers, and the interdisciplinary team to ensure seamless communication and care delivery.

Qualifications

Applicants must hold a valid NYS RN License and registration. A Bachelor of Science in Nursing is required, along with a minimum of three (3) years of nursing experience. Experience in geriatrics and managed long-term care is preferred. Bilingual proficiency in Russian, Chinese, or Spanish is essential based on operational needs.

Compensation

The annual salary for this position ranges from $95,000 to $110,000.
