As a skilled Care Coordinator, you will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family.
Key Responsibilities:- Conduct thorough assessments of potential new members to determine eligibility for the program.
- Develop and implement personalized care plans to meet the unique needs of each member.
- Collaborate with the Intake/Outreach Team and the Clinical Team to ensure seamless provision of services to new members.
- Communicate effectively with members, family, referral sources, and physicians to ensure a smooth transition to managed long-term care.
- Current New York State Registered Nurse License required.
- Bachelor's degree in Nursing required.
- One (1) year experience in an acute care setting required.
- Must have UAS-NY experience.
- Home care and management experience preferred.
- MS Office required.
- Effective oral, written, communication, and interpersonal skills required.
$95,000 - $105,000 Annually
