MLTC Intake Nurse – RN Bilingual Spanish (Brooklyn, NY)

As a skilled Care Coordinator, you will be responsible for assessing potential new members for eligibility for the program and for explaining the program to them and their family.

Key Responsibilities:
  • Conduct thorough assessments of potential new members to determine eligibility for the program.
  • Develop and implement personalized care plans to meet the unique needs of each member.
  • Collaborate with the Intake/Outreach Team and the Clinical Team to ensure seamless provision of services to new members.
  • Communicate effectively with members, family, referral sources, and physicians to ensure a smooth transition to managed long-term care.
Requirements:
  • Current New York State Registered Nurse License required.
  • Bachelor's degree in Nursing required.
  • One (1) year experience in an acute care setting required.
  • Must have UAS-NY experience.
  • Home care and management experience preferred.
  • MS Office required.
  • Effective oral, written, communication, and interpersonal skills required.
Compensation and Benefits:

$95,000 - $105,000 Annually
