About the Role

We are seeking a highly skilled and compassionate Care Specialist to join our team at Southeastern Integrated Care LLC. As a key member of our care coordination team, you will play a vital role in providing exceptional care and support to our clients.

Key Responsibilities
  • Develop and Implement Care Plans: Collaborate with clients, families, and healthcare professionals to create personalized care plans that meet the unique needs and goals of each individual.
  • Coordinate Care Services: Arrange and coordinate various care services, including medical appointments, therapy sessions, and home care, to ensure seamless and efficient care delivery.
  • Advocate for Clients: Act as a strong advocate for our clients, ensuring their rights and interests are protected and their voices are heard.
  • Communicate Effectively: Develop and maintain effective relationships with clients, families, and healthcare professionals through clear and compassionate communication.
  • Monitor and Evaluate Care: Regularly monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
  • Collaborate with Team Members: Work closely with other care specialists, healthcare professionals, and support staff to ensure comprehensive and coordinated care.
  • Document and Record Keeping: Maintain accurate and detailed records of client interactions, care plans, and service delivery, ensuring compliance with regulatory requirements.
  • Participate in Continuous Learning: Engage in ongoing education and training to stay up-to-date on best practices, industry developments, and regulatory requirements.
Requirements
  • Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience in care coordination or a related field.
  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, strong problem-solving and critical thinking skills, and a commitment to providing exceptional care and service.
  • Certifications and Licenses: Valid driver's license and current certification in CPR/First Aid, if required by state or local regulations.
Work Environment

The work environment for this role will be primarily in the client's home and in the community, with occasional travel to healthcare facilities and other locations. The ideal candidate will be able to work in a fast-paced environment, prioritize tasks effectively, and maintain a high level of professionalism and compassion in all interactions.
