  • Communicate Effectively: Maintain open and transparent communication with clients, families, and healthcare professionals to ensure everyone is informed and engaged in the care process.
  • Monitor Progress: Regularly monitor client progress, identifying areas of improvement and making adjustments to care plans as needed.

Requirements

  • Skills and Abilities: Excellent communication and interpersonal skills, ability to work independently and as part of a team, strong problem-solving and critical thinking skills, and a commitment to providing exceptional care and support.
  • Certifications and Licenses: Valid driver's license and current certification in care coordination or a related field (if applicable).
Work Environment

The work environment for this role is primarily in the client's home and in the community, with occasional travel to meetings and training sessions.

Physical Demands

The physical demands of this role include standing, walking, and driving, with occasional lifting and bending. The ability to work in a fast-paced environment and adapt to changing circumstances is essential.
