- Coordinate Services: Arrange and coordinate services such as medical appointments, therapy sessions, and home care to ensure seamless care delivery.
- Advocate for Clients: Act as a strong advocate for our clients, ensuring their rights and interests are protected and their voices are heard.
- Communicate Effectively: Maintain open and transparent communication with clients, families, and healthcare professionals to ensure everyone is informed and engaged in the care process.
- Monitor Progress: Regularly monitor client progress, identifying areas of improvement and making adjustments to care plans as needed.
