Job Summary

At Southeastern Integrated Care LLC, we are seeking a highly skilled and compassionate Intensive In-Home Qualified Professional/Case Manager to join our team. As a key member of our care coordination team, you will play a vital role in providing high-quality, person-centered care to individuals with mental health, developmental disabilities, and substance abuse needs.

Key Responsibilities
  • Develop and Implement Person-Centered Plans: Collaborate with individuals, families, and healthcare providers to create and update comprehensive, individualized plans that address their unique needs and goals.
  • Coordinate Care Services: Arrange and coordinate services, including medical appointments, therapy sessions, and community-based activities, to ensure seamless care transitions and optimal outcomes.
  • Advocate and Support: Empathetically advocate for individuals and their families, providing emotional support and connecting them with community resources to promote independence and well-being.
  • Document and Communicate: Accurately document all interactions, services, and outcomes, and effectively communicate with team members, healthcare providers, and stakeholders to ensure continuity of care.
  • Supervise and Train: Provide guidance and support to junior staff members, promoting professional development and ensuring high-quality care delivery.
Requirements
  • Education and Experience: Bachelor's degree in a human services field or related field, with a minimum of 2 years of experience working with individuals with mental health, developmental disabilities, and substance abuse needs.
  • Skills and Abilities: Excellent communication, documentation, and interpersonal skills; ability to work independently and as part of a team; flexibility in responding to changing needs and priorities.
  • Certifications and Licenses: Valid NC driver's license and personal vehicle insurance coverage; current license or certification in a relevant field (if applicable).
Work Environment

The work environment for this position is primarily in the client's home and community, with occasional travel to healthcare facilities and other locations. The ideal candidate will be able to work effectively in a fast-paced, dynamic environment and adapt to changing circumstances.

Physical Demands

The physical demands of this position include standing, walking, and driving, with occasional lifting and moving of up to 20 pounds. The ability to drive and operate a personal vehicle safely is essential.
