Pembroke Center is seeking a full-time Registered Nurse - MDS Specialist to join our dedicated team. This position operates Monday through Friday from 8 AM to 5 PM. Candidates with prior experience are highly preferred.

Role Overview:
The MDS Specialist - RN is accountable for the timely and precise completion of the MDS assessment tool. This role involves reviewing care plans and their implementation, focusing on our patients' physical, cognitive, and social needs to ensure adherence to our exceptional care standards.

Key Responsibilities:
* Oversee the management and tracking of all Medicare/Medicaid case-mix documentation to guarantee appropriate reimbursement for services rendered within the nursing facility.
* Conduct concurrent MDS evaluations to ensure the attainment of maximum allowable RUG categories.
* Collaborate with nursing, dietary, social services, restorative, rehabilitation, and physician teams to secure proper reimbursement.

Qualifications:
* Must be a graduate of an accredited School of Nursing with current Registered Nurse (RN) licensure. LPNs with MDS experience may be considered.
* A minimum of one year of experience in long-term care nursing is required.
