Job Summary

New England Life Care is seeking a highly skilled and compassionate Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in ensuring the seamless coordination of care for our patients receiving home infusion therapy.

Key Responsibilities
  • Accurately compile patient profiles and distribute them to the healthcare team for processing
  • Communicate effectively with pharmacy, reimbursement, and delivery staff regarding new referrals and patient issues
  • Work collaboratively with the Liaison/Care Coordination team to ensure efficient discharge
  • Document all patient-related information and conversations in the patient's clinical record
  • Communicate changes in patient supply requirements to Pharmacy and Patient Service Representative
Requirements
  • Associate of Arts degree preferred
  • Previous medical office experience a plus
  • Medical terminology knowledge preferred
