Job Title: SOURCE Case Manager

At The Arc Southwest Georgia, we are seeking a highly motivated and compassionate SOURCE Case Manager to join our team.

Job Summary:

The SOURCE Case Manager is responsible for providing enhanced primary care and case management services to members assigned to their caseload. This role requires a strong understanding of community resources, healthcare systems, and social services.

Key Responsibilities:

  • Collect and review monthly reports to ensure timely completion and accuracy.
  • Maintain and monitor members' files and records, ensuring up-to-date information and follow-up on reviews as needed.
  • Obtain medical information for all members and assist with medication acquisitions.
  • Monitor members' overall health status and wellbeing, addressing medical and social issues promptly.
  • Assist members with doctor's appointments and follow physicians' recommendations.
  • Attend community/agency meetings regarding SOURCE members as needed.
  • Assist members with SSI and Medicaid benefits maintenance.
  • Collaborate with SOURCE nurses on specific medical/health issues of concern.
  • Report all issues of concern to the SOURCE Director.
  • Collaborate with participating physicians and medical facilities to ensure wrap-around case management of members' health needs.
  • Utilize Therap to input data on members by the end of the service month.
  • Provide and coordinate related services to members, including transportation, medical appointments, counseling, information and referral, and other duties as assigned.

Qualifications:

Required: Bachelor's degree in Sociology, Criminal Justice, Psychology, Social Work, Special Education, or related field. Must have experience working in and with community agencies, with families, and preferably with individuals who have disabilities.
