Licensed Practical Nurse Home Health Per Diem

CenterWell Home Health is seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home health care to patients in need. As a Home Health LPN, you will play a vital role in delivering compassionate and personalized care to patients in the comfort of their own homes.
Job Responsibilities- Provide professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations, as well as CenterWell Home Health policies and procedures.
- Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and preventative care.
- Confer with RN Supervisors to ensure seamless care coordination and make necessary adjustments to patient plans as needed.
- Maintain accurate and detailed records of patient interactions, including visit notes and progress updates.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive patient care.
- Participate in ongoing education and training to stay current on best practices and industry developments.
- Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
