**Key Responsibilities:**

* Coordinate health education and services with physicians, staff, and other healthcare providers.
* Provide group or individual patient education and information to manage chronic health conditions.
* Participate in the planning, implementation, and evaluation of diabetes self-management education.
* Notify supervisor or physician of patient's reaction to drugs, treatments, and significant incidents.
* Initiate assessment and education according to the individualized needs of the patient, as prescribed by physicians and/or policy including patient and family instruction.
* Prepare and disseminate educational and informational materials.
* Participate in the development of programs designed to enhance health education.
* Market services of health education department and/or clinic.
* Participate in multidisciplinary team meetings.
* Maintain educational resources.
* Perform other duties as assigned.

**Qualifications:**

* Bachelor's degree in Food and Nutrition, Dietetics, Nutritional Sciences, Human Nutrition, or related major; and completion of coordinated program or dietetic internship of not less than 900 hours under the supervision of a Registered Dietitian.
* Experience in chronic disease management programs preferred.
* Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
* Ability to write reports and business correspondence.
* Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
* Fluent in English required; Spanish bilingual preferred.

**Certifications and Licenses:**

* Licensed and Registered, or registration eligible, Dietitian in the state of Texas.
* Certified Diabetes Educator preferred.
* Or must be certified as Diabetes Educator within two years of employment.

**Computer Skills:**

* Demonstrates working knowledge of the Internet, Outlook, PowerPoint, Word, Excel, and electronic health records software.
