Job SummaryThis is a fantastic opportunity for a compassionate and skilled RN to deliver high-quality patient care in the comfort of patients' homes. As a Registered Nurse with SunCrest Home Health, you will be responsible for providing personalized nursing care, assessing patients' needs, developing care plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.
In this role, you will have the opportunity to work independently, utilizing your clinical expertise to make decisions that improve patient care. You will also have the chance to build meaningful relationships with patients, their families, and other healthcare professionals, making a positive impact on the lives of those you serve.
About the RoleThe Registered Nurse in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. This role involves coordinating total plan of care with other health care professionals involved in care and helping to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources.
You will make the initial and/or comprehensive nursing evaluation visit, determine primary focus of care, develop the plan of care within state-specific guidelines, and submit accurate, complete, and timely documentation in collaboration with physician orders. Additionally, you will perform patient comprehensive assessments at designated time points and develop the appropriate plan of care.
Requirements include current RN licensure in the state of practice, current CPR certification, and a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
What We Offer- A competitive salary range: $65,000 - $85,000 per year, based on experience.
