Hospice Care Specialist

At (434300) Medi Home Health & Hospice, we seek a compassionate and skilled Certified Nursing Assistant to join our team. As a Hospice Aide, you will provide essential support services to our patients and their families, ensuring their physical and emotional comfort.
Key Responsibilities:
- Deliver personal care and activities of daily living as per the patient's plan of care.
- Implement safety principles and proper body mechanics to perform personal and supportive care tasks.
- Prepare and provide adequate nutrition and fluid intake, following medically prescribed diets under supervision.
- Monitor and report changes in patient condition or service expectations to skilled nurses via phone and document in the Electronic Medical Record (EMR).
Requirements:
- High school diploma or General Education Degree (GED) required.
- State-recognized Certified Nursing Assistant course completion.
- Valid Certified Nursing Assistant certification.
- Valid driver's license and company-required auto-liability insurance.
