This role focuses on delivering high-quality nursing care services in accordance with physician orders. The Clinical Nurse Specialist will assess, plan, implement, and evaluate the care needs of a designated group of patients or provide nursing care and identified clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
The majority of time is spent overseeing the design, implementation, or delivery of processes, programs, and policies using specialized knowledge and skills typically acquired through advanced education. A senior-level role that requires advanced knowledge of the job area, typically obtained through advanced education and work experience.
Responsibilities- Provides leadership within nursing by providing direction and priority setting in regards to patient care.
- Develops and maintains collaborative relationships between all members of the Health Care Team, inclusive of administrative, clinical, research, and inpatient partners, thus producing high-performing, patient-focused teamwork.
- Acts as a resource for all staff in regards to education, planning, relocating, and development of evidence-based, best-practice clinical programs pertaining to the specialty or primary care patient population.
- Mentors and actively participates in overseeing all clinical support staffing in the ambulatory clinical setting; delegates responsibilities appropriately and within scope.
- In conjunction with satellite clinical and administrative leadership, participates in coordinating any transition of patient care from the satellite setting to another in collaboration with and maintaining open communication with a wide range of interdisciplinary team members with the goal of achieving an exceptional patient experience.
- Identifies high-risk patients and works with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
- Promotes shared decision-making with patients in regards to healthcare; educates the patient and/or family regarding the patient's clinical condition, treatment, and their role in their healthcare decisions.
- Utilizes a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
- Collaborates and actively engages with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.
- Demonstrates compliance with regulatory requirements, i.e., MIPS, fall assessment.
- Participates in satellite quality and safety initiatives as indicated.
