Clinical Nurse Specialist

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse to provide high-quality clinical services to patients in various settings within the community. As a key member of our interdisciplinary team, you will be responsible for delivering patient-centered care, promoting independence, and optimizing quality of life for our patients.
Responsibilities- Provide skilled nursing care to patients in their homes, focusing on assessment, teaching, treatment, and documentation.
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing strategies for care.
- Develop and maintain effective relationships with patients, families, and caregivers, ensuring open communication and addressing concerns in a timely manner.
- Participate in ongoing education and training to stay current with best practices, new technologies, and emerging trends in home care.
- Contribute to the development and implementation of policies, procedures, and standards of practice that promote high-quality patient care and safety.
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
- Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
- Strong interpersonal and customer service skills, with the ability to work with a diverse population.
- Effective communication and documentation skills, with the ability to work with various computer software and hand-held patient care devices.
