Job Profile Summary

This role focuses on delivering high-quality nursing care services in accordance with physician orders. As a key member of the healthcare team, you will be responsible for assessing, planning, implementing, and evaluating the care needs of a designated group of patients. Your expertise will be utilized to provide professional nursing care, identify opportunities for improvement, and collaborate with interdisciplinary team members to achieve exceptional patient outcomes.

Key Responsibilities:
  1. Provide leadership and direction to nursing staff, prioritizing patient care and ensuring seamless coordination with other healthcare professionals.
  2. Develop and maintain collaborative relationships with administrative, clinical, research, and inpatient partners to foster high-performing, patient-focused teamwork.
  3. Act as a resource for staff, providing education, planning, and development of evidence-based clinical programs to enhance patient care.
  4. Mentor and oversee clinical support staffing in the ambulatory setting, delegating responsibilities appropriately and within scope.
  5. Participate in coordinating patient care transitions between satellite and other settings, maintaining open communication with interdisciplinary team members.
  6. Identify high-risk patients and work with appropriate services to achieve optimal patient care and outcomes.
  7. Promote shared decision-making with patients, educating them on their clinical condition, treatment, and role in healthcare decisions.
  8. Utilize telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
  9. Collaborate with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access, and cost savings.
  10. Demonstrate compliance with regulatory requirements, such as MIPS and fall assessment.
