Clinical Nurse Specialist

Job Summary
Tufts Medicine is seeking a skilled Clinical Nurse Specialist to join our team. In this role, you will provide professional nursing care to patients, assess, plan, implement, and evaluate the care needs of a designated group of patients, and perform clinical support tasks within the scope of practice for achievement of the patient's plan of care as directed.
Key Responsibilities
- Provide leadership within nursing by providing direction and priority setting in regards to patient care.
- Develop and maintain collaborative relationships between all members of the Health Care Team.
- Act as a resource for all staff in regards to education, planning, relocating and development of evidence-based, best practice clinical programs.
- Mentor and actively participate in overseeing all clinical support staffing in the ambulatory clinical setting.
- Identify high risk patients and work with the appropriate services and interdisciplinary team members to achieve the best possible patient care and outcomes.
- Promote shared decision making with patients in regards to healthcare.
- Utilize a variety of telecommunications technologies to assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes.
- Collaborate and actively engage with the care team and practice leadership to identify opportunities and solutions to improve quality, patient access and cost savings.
Requirements
- Bachelor's degree in Nursing (BSN).
- Registered Nurse (RN) license.
- Basic Life Support (BLS) certification.
