Job Summary

We are seeking a highly skilled and compassionate LPN to join our team as an Intake Specialist at New York Health. As a key member of our multidisciplinary team, you will play a vital role in providing exceptional patient care and ensuring a seamless experience for our patients.

Key Responsibilities
  • Provide high-quality patient care, including vital sign measurements, patient interviews, and medication education.
  • Update and maintain accurate and detailed electronic patient records.
  • Assist physicians with exams and procedures, including phlebotomy and collection of lab specimens.
  • Administer and document medication under a physician's order, adhering to strict medication safety protocols.
  • Collaborate with the healthcare team to ensure effective communication and coordination of patient care.
  • Participate in ongoing quality improvement initiatives to enhance patient satisfaction and outcomes.
Requirements
  • Current NYS LPN license.
  • Basic Life Support (BLS) certification.
  • Experience in a lead role or demonstrated potential to act in a lead capacity.
  • Demonstration of consistent professional conduct and meticulous attention to detail.
  • Possession of excellent verbal and written communication skills, as well as excellent interpersonal skills with patients, staff, and other healthcare professionals.
  • Proficiency in ONCOemr.
