Position Title: Case Manager

Department: Supportive Housing Services

Classification: Non-Exempt

Location: YMCA/Westhab

Supervisor: Service Coordinator

Compensation: $40,000 - $45,000 Annually

Position Summary: The Case Manager plays a crucial role in ensuring that all clients enrolled in the program receive comprehensive assessments and are directed to suitable services. This position is responsible for delivering case management and supportive services to residents, as well as participating in essential program meetings with partner agencies.

Key Responsibilities:

  • Manage an individual caseload for single adult residents living in SROs.
  • Collaborate with the Service Coordinator to develop tailored service plans for all residents.
  • Engage with residents to facilitate the creation of treatment plans, vocational training, continuing education, and in-house meeting schedules while documenting compliance.
  • Conduct outreach to shelters, perform intake interviews, and oversee the move-in process for new residents, ensuring full occupancy.
  • Maintain communication with all partner agencies to support resident compliance with Individualized Learning Plans (ILPs).
  • Prepare reports and conduct necessary analyses as required.
  • Organize and facilitate house meetings.
  • Schedule inspections of rooms/units on a weekly or bi-weekly basis.
  • Report any property or unit deficiencies to the Service Coordinator.
  • Assist in monitoring monthly rent payments, manage delinquent accounts, and follow up as directed by the Service Coordinator.
  • Perform additional duties as assigned.

Qualifications:

An Associate or Bachelor's Degree in Social Work, Psychology, or a related field is required, along with prior experience in social services, preferably with populations affected by substance abuse, mental health issues, and homelessness. Bilingual (Spanish) skills are advantageous. A valid driver's license and access to a vehicle are necessary. Some evening and weekend availability is required. Excellent written and verbal communication skills are essential.
