Job Title: Patient Outreach and Transition Support Specialist

We are seeking a highly skilled Patient Outreach and Transition Support Specialist to join our team at Actalent. In this role, you will be responsible for engaging with patients who are overdue for appointments, providing friendly and empathetic support, and encouraging them to schedule appointments. You will also be responsible for reaching out to patients whose physician has left the practice, understanding their new plan of care, and guiding them to physicians similar to their previous provider.

Key Responsibilities:
  • Contact patients who are overdue for appointments and encourage them to schedule appointments
  • Reach out to patients whose physician has left the practice and guide them to new physicians
  • Verify and update patient histories, medical records, and relevant information using EMR/CRM systems
  • Identify and remove records of deceased patients or those who have moved out of coverage areas
  • Collaborate with other clinics to assist with appointment scheduling and logistics
Requirements:
  • EMR experience
  • Primary care experience
  • Medical terminology knowledge
  • Healthcare administration skills
  • Medical office specialist skills
Work Environment:

This is a fully on-site position in Kansas City, Missouri. You will sit out of Research Medical Center, KCMO, in an office setting environment. You will have daily, weekly, and monthly goals for metrics with inbound/outbound calls, setting appointments, and updating patient profiles. We will review data monthly to ensure it is making an impact with success.
