Companion Case Management Aide

Era Living is looking for dedicated Companion Case Management Assistants to join our Resident and Family Supportive Programs team. We offer flexible scheduling options, allowing you to work on an on-call or scheduled basis for up to 20-25 hours per week based on the needs of our residents.
Key Responsibilities:
Under the guidance of the Resident and Family Supportive Programs Director and in partnership with community counselors, this position involves providing essential case management services to residents who may be facing challenges such as memory loss, loneliness, anxiety, grief, or organizational difficulties. Your tasks may include:
- Assisting in organizing living spaces and reducing clutter
- Engaging in reading activities
- Facilitating puzzles, crosswords, Sudoku, and similar activities
- Accompanying residents on walks
- Joining residents in social activities

Qualifications:
- Ability to work collaboratively within a multidisciplinary team
- Strong interpersonal skills to facilitate effective communication with residents, family members, and other stakeholders
- Capability to foster a positive atmosphere and represent Era Living professionally, maintaining appropriate boundaries and discretion
- Proficiency in using computers, including Word, Excel, Outlook, and internet applications
- Access to a personal computer/tablet or smartphone with internet capabilities and the ability to utilize email and calendar functions
- Availability to conduct 4 visits per month
- Proficient in reading, writing, and communicating in English
- Ability to pass a DSHS background check
Education and Experience:
- High School diploma or GED required; Bachelor's Degree in human services, social work, gerontology, or related fields preferred
- Experience working with senior populations preferred
- First Aid and CPR certification or the ability to obtain within 30 days of hire
- Completion of Caregiver Dementia and Caregiver Mental Health training within 120 days of hire
