Job Summary
Neighborhood House is devoted to ensuring that low-income, immigrant, and refugee seniors and individuals with disabilities can age in their homes, receiving culturally sensitive services that enhance their quality of life. The primary responsibility of this role is to deliver case management services to approximately 20-30 clients, conduct in-home functional evaluations, and coordinate in-home caregiving for elderly and disabled clients receiving Medicaid-funded personal care and additional services from the Department of Social and Health Services (DSHS). The Case Aide will collaborate with the administrative and clinical teams of the MAC TSOA and LTC programs to manage data effectively. This position will report directly to the LTC Administrative Supervisor.
The ideal candidate will thrive in a dynamic environment and possess experience working with diverse populations.
Responsibilities
- Manage a caseload of 20-30 clients, conducting screenings, assessments, home visits, and fulfilling all program compliance requirements as outlined in the ALTSA Long Term Care Manual.
- Utilize DSHS and ADS data systems to generate reports that track multiple deadlines; communicate efficiently with clients, case managers, care coordinators, and providers to ensure compliance with DSHS regulations.
- In collaboration with the clinical team, coordinate necessary documentation, authorizations, and ordering processes for medical equipment requests from clients, ensuring adherence to DSHS guidelines. Foster relationships with providers to streamline this process.
- Participate in assigned training and professional development opportunities; prepare training sessions for administrative and/or clinical teams as needed.
- Provide language assistance to the team as required.
- Translate materials and proofread documents in assigned languages as needed.
- Perform various administrative duties, including managing e-fax communications, handling mail, document verification, and monitoring phone calls.
- Execute other duties as assigned.
Qualifications
Minimum Qualifications
- Associate degree in social work, social sciences, health, human services, or a related field. Relevant experience may substitute for formal education.
- Two years of experience in social services, health, or a similar fast-paced environment with a focus on diverse populations. A combination of education and experience may be considered.
- Proven ability to navigate complex Federal and State regulations and complete extensive technical documentation.
- Highly organized with strong attention to detail.
- Demonstrated ability to provide excellent customer service in a fast-paced setting.
- Effective communication skills, both verbal and written, with individuals from various cultural backgrounds.
- Commitment to advancing racial equity and social justice as part of Neighborhood House's mission to become a fully inclusive, multicultural, anti-racist organization.
- Reliable transportation for occasional travel to client homes and between work sites.
- Proficiency in key computer applications (MS Office/Word/Excel/PowerPoint/Access).
Desired Qualifications
- Bachelor's degree in Social Work, Behavioral or Health Sciences, or a related field.
- Familiarity with client services-tracking databases or related database navigation.
- Experience in case management within a multicultural environment.
- Knowledge of elderly or disability issues and resources that assist these populations in maintaining independence and quality of life.
- Fluency in Spanish.
Required Credentials
- Successful completion of Neighborhood House background check requirements.
- Position requires bi-annual criminal background re-check in Washington State.
