We are seeking a skilled and compassionate RN MDS Case Manager to join our team at Providence Mount St. Vincent. As a key member of our care management team, you will play a vital role in ensuring the highest quality of care for our patients.

Key Responsibilities
  • Conduct comprehensive assessments of patients' medical, functional, and psychosocial needs
  • Develop and implement individualized care plans to meet patients' needs and goals
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Monitor and evaluate patients' progress, making adjustments to care plans as needed
  • Provide education and support to patients and their families to promote self-management and independence
Requirements
  • Current Washington State Registered Nurse License
  • Two years of experience in Long-Term Care (LTC), Transitional Care Unit (TCU), or equivalent experience
  • Bachelor's Degree in Nursing (preferred)
