Position Summary:We are looking for a committed and empathetic Client Services Coordinator to become part of our organization. Key Responsibilities:
Under the general supervision of the unit Supervisor, this individual will provide service coordination for eligible participants who qualify for Medicaid under Community Health Choices. Responsibilities include:
- Conducting comprehensive assessments to identify needs.
- Facilitating connections to necessary services.
- Creating and maintaining service plans that reflect participant strengths and needs.
- Monitoring ongoing adherence to plans and participant satisfaction.
- Communicating effectively with participants and their support teams.
- Ensuring participants maintain Medicaid eligibility.
- Documenting all services rendered accurately and promptly.
- Providing continuous support and crisis intervention as needed.

Qualifications:
(1) Registered Nurse (RN); or (2) a bachelor’s degree in Social Work, Psychology, or related fields with practical experience; or (3) a minimum of three (3) years of experience in a social service or healthcare setting.
Employment Prerequisites:
Possession of a valid driver’s license, automobile insurance, acceptable motor vehicle record, and necessary clearances.
Work Environment:
Comfortable working in an open office setting and utilizing virtual communication tools. Travel within the coverage area is required and may occur under varying conditions. Ability to lift at least 15 pounds is necessary.
