Position Summary:

Key Responsibilities:

Under the general supervision of the unit Supervisor, this role involves providing service coordination for eligible participants who qualify for Medicaid under Community Health Choices. Service Coordinators are tasked with delivering ongoing service coordination, including continuous monitoring to ensure individual health, safety, and wellness; connecting to community resources; communicating vital information to participants; and developing and maintaining service plans that outline participant strengths, needs, and services.

Core Duties:

Assess needs and conduct comprehensive evaluations.

  • Engage in ongoing evaluations of participants, including completing thorough assessments.

Facilitate access to services.

  • Ensure participants are referred to necessary and/or approved services.

Develop and maintain service plans.

  • Create and uphold plans that reflect participant strengths and needs.

Monitor progress.

  • Conduct ongoing monitoring activities to ensure adherence to plans, satisfaction with services, and the ongoing safety and well-being of participants.

Communicate and collaborate effectively.

  • Share priorities and information with participants and their designated team members.

Verify eligibility.

  • Service coordinators ensure that assigned participants maintain Medicaid eligibility.

Documentation.

  • Keep accurate and current records, including writing service notes for all services provided.

Provide continuous support and crisis intervention.

  • Service coordinators are responsible for fostering positive working relationships with participants and their identified team members to ensure that necessary ongoing support is delivered.

Qualifications:

(1) Registered Nurse (RN); or (2) possess a bachelor's degree in Social Work, Psychology, or related fields with practical experience; or (3) have a minimum of three (3) years of experience in a social service or healthcare-related setting.

Employment Prerequisites:

Valid automobile and insurance, valid driver's license, acceptable motor vehicle record check, Act 33 and Act 34 clearances, FBI fingerprint clearance, social security card, immigration and naturalization (I-9), and drug screening clearance.

Service Coordinators working within the Nursing Home project must complete TB testing.

Work Environment:

Comfortable working in an open office setting and communicating through virtual technology. Travel throughout the coverage area is required and may occur in adverse conditions. Must be able to lift at least 15 pounds.
