Overview

This exempt role involves the practice of critical care medicine and the comprehensive management of inpatient medical services under the supervision of a licensed physician, in accordance with New York State regulations. The Physician Assistant/Nurse Practitioner operates with a degree of independence in clinical decision-making and provides a wide array of diagnostic and therapeutic interventions.

Key Responsibilities Include:

  • Adhering to the performance standards, policies, and behaviors established by the organization.
  • Collecting patient medical histories, performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and managing treatment plans in collaboration with physicians.
  • Offering consultative services for patients in need of critical care from various departments, including the emergency room and post-operative care.
  • Engaging in daily rounds to formulate and assess treatment strategies for patients within the critical care unit.
  • Communicating treatment objectives and other significant matters with the families of critically ill patients.
  • Responding promptly to any patient exhibiting acute changes in their clinical condition.
  • Coordinating necessary treatments, interventions, diagnostic tests, or procedures required for patient care.
  • Facilitating appropriate referrals to specialists as needed.
  • Providing timely updates to the on-call intensivist regarding patient progress, significant changes, or challenges faced in care.
  • Performing medical procedures within the authorized scope of practice.
  • Advising patients and families on healthcare matters, including discussions surrounding end-of-life care.
  • Documenting patient care and information accurately in medical records, which may also involve maintaining departmental databases for research and quality assurance.
  • Demonstrating proficiency in anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis.
  • Exhibiting skill in collecting and analyzing physiological, psychological, and socioeconomic data related to patients.
  • Effectively evaluating patient issues in person or via telephone, providing suitable advice, interventions, or referrals.
  • Making informed decisions within the framework of PA practice.
  • Facilitating the care and management of patients admitted to the critical care units.
  • Carrying out additional duties as assigned.

Qualifications & Experience

For Nurse Practitioners:

  • Master’s Degree in Nursing from an accredited institution (strongly preferred).
  • Valid New York State license as a Registered Professional Nurse.
  • Current certification as a Nurse Practitioner in New York State (Adult/Geriatric).
  • Maintenance of a collaborative practice agreement.
  • Active National Certification.
  • Certification in Advanced Cardiac Life Support (ACLS).

For Physician Assistants:

  • Current New York State license as a Physician Assistant.
  • Active National Certification.
  • Current White Plains Hospital Physician Assistant DOP.
  • Master’s degree from an accredited institution (strongly preferred).
  • Certification in Advanced Cardiac Life Support (ACLS).

Proficiency in all procedures, techniques, and skills as delineated in the privileges granted.

Core Competencies

Required Procedural Skills:

  • Insertion of central lines (triple-lumen or introducer) and dialysis catheters.
  • Insertion of arterial lines (radial, femoral, axillary).
  • Placement of nasogastric (NGT) and orogastric tubes (OGT).
  • Insertion of Foley catheters.
  • Removal of surgical drains, dressings, and staples.
  • Participation in training programs if lacking procedural skills.

Knowledge Base Requirements:

  • Management of various shock states.
  • Goal-directed fluid resuscitation techniques.
  • Administration of vasopressors and inotropic agents.
  • Proficiency in hemodynamic monitoring.
  • Ventilator management and weaning protocols.
  • Understanding of sedation and paralytic agents, including monitoring techniques.
  • Daily sedation interruption and weaning evaluations.
  • Continuous venovenous hemodialysis (CVVHD) management.
  • Management of electrolyte and acid-base imbalances.
  • Interpretation of arterial blood gases (ABG).
  • Monitoring of intracranial pressure (ICP).
